Our Financial Counselors will work with you to determine whether you qualify for our Sliding Fee Discount Program, what other resources may be available to you, and how to create a payment plan that works within your budget. We can also offer referrals to other programs, help you navigate the Medicaid system, and assist with paperwork and translation.
Please note: Medicaid is ALWAYS welcome at FHC. Medicaid patients are never turned away.
According to federal legislation enacted in 2023, Michigan Medicaid beneficiaries will be required to renew their coverage starting in June 2023. Ensure you remain eligible for Medicaid by keeping your address, phone number, and email address current.

Supplemental foods, health referrals and nutrition education for low-income pregnant, breast-feeding and postpartum women and children under age 5.
The MI Care Team service model helps chronically ill Medicaid and Healthy Michigan Plan beneficiaries better manage their conditions. Qualified patients are those with depression/and or anxiety plus one or more of these conditions: heart disease, chronic obstructive pulmonary disease, hypertension, diabetes or asthma.
