From financial aid to program referrals, we do what it takes to get you feeling your best.

We welcome every person through our doors regardless of that person’s ability to pay. We also help people cultivate the ability to contribute to his or her own health care in a way that is personally empowering. 

Our Financial Counselors will work with you to determine whether you qualify for our Sliding Fee Discount Program, what other resources may be available to you, and how to create a payment plan that works within your budget. We can also offer referrals to other programs, help you navigate the Medicaid system, and assist with paperwork and translation.

Please note: Medicaid is ALWAYS welcome at FHC. Medicaid patients are never turned away.

Patient Financial Counseling

Please ask to speak with a Financial Counselor if you are having difficulty paying your bill, have lost your job, are uninsured, or you’re simply worried about budgeting for your health care. Our calm, kind and caring staff is here to help you find solutions for healthy living!

Outreach & Enrollment

FHC assists patients with enrollment in Affordable Care Act insurance plans and Healthy Michigan/Medicaid coverage. Call 269-349-2641 for assistance.

Medical Social Work

Wrap-around services and patient navigation assistance for people who are struggling with many complex issues.

Pain Review & Referral

Helping patients manage chronic pain. Our specially trained Pain Review Team works with your primary provider to help you explore treatment options.

Women, Infants & Children (WIC)

Supplemental foods, health referrals and nutrition education for low-income pregnant, breast-feeding and postpartum women and children under age 5.

MI Care Team​

The MI Care Team service model helps chronically ill Medicaid and Healthy Michigan Plan beneficiaries better manage their conditions. Qualified patients are those with depression/and or anxiety plus one or more of these conditions: heart disease, chronic obstructive pulmonary disease, hypertension, diabetes or asthma.
